Limited Partnership
- ID for all signers
- Assumed Name Certificate DBA (If applicable)
- Certificate of Formation
- Certificate of Filing
- Partnership Agreement

Limited Liability Company (Includes PLLC)
- ID for all signers
- Articles of Incorporation / Certificate of Formation
- Certificate of Incorporation / Certificate of Filing
- Assumed Name Certificate DBA (If applicable)

Corporate Accounts (includes PC):
- ID for all signers
- Articles of Incorporation / Certificate of Formation
- Certificate of Incorporation / Certificate of Filing

Clubs / Civic Organizations / Associations / EIN (Non-Profit), includes Professional Associations (PA)
